COMMUNITY HEALTH ADVOCATE (CHA)
Community Connections has two highly qualified Community Health Advocates to provide services to our members. They have a wealth of knowledge in nursing. They support with education, home safety, listen to concerns, connect with local resources, assist with doctor visits and more. This qualified team is available to attend medical appointments, help with scheduling and understanding medical tests and medications and provide in-home safety checks and more.

COMPANION / HOME ASSISTANCE
This special role provides support to our members and their caregivers through companionship (can help to determine what brings joy as every person is unique), meal preparation support and nutritional education, home safety and simple errands.

YELLOW DOT PROGRAM
Community Connections has partnered with the Chautauqua County Sheriff's Department to bring the Yellow Dot program to Findley Lake. This program is designed to help first responders provide immediate care whether in your home or responding to a car accident. Yellow Dot Kits are available in our office.

REFLECTIVE HOUSE NUMBERS
Community Connections was awarded a grant from the Johnson Foundation in Jamestown, NY. Community Connections decided to use a portion of the grant towards the purchase of reflective house number signs for the safety of its residents. This program was in collaboration with the Chautauqua County Sheriff's Department. It is being continued through the donation from the Findley Lake Fire Department's Women's Auxiliary.

VOLUNTEER PROGRAM — TO SERVE OUR COMMUNITY
Community Connections’ volunteers can assist with services such as shopping, retrieving mail, running errands, or transportation to and from your doctor's office.

R.U.O.K.
This program is a free service that allows a daily check on your well-being. This automated phone system service is administered by the Chautauqua County Sheriff's office and sponsored by Community Connections. The system allows you to receive a FREE automated phone call a day to assure you are "OK."

HEALTH INSURANCE INFORMATION COUNSELING (HIICAP)
Community Connections has sponsored two of its directors to become certified Health Insurance Information Counselors. This allows Community Connections to serve as an information source for anyone with questions about their Medicare insurance.

MEDICARE SAVINGS PROGRAM
Your Medicare Part B fee is automatically deducted from your Social Security check. Your monthly social security payment could be returned to you if you qualify.

LEGAL ASSISTANCE
Free legal assistance is available to seniors at least 60 years of age. This program is offered in conjunction with the Office for Aging via a New York State grant.

MEDICAL EQUIPMENT LENDING LIBRARY
Borrow walkers, crutches and canes that are in very good condition. Donations currently being accepted.

SHOPPING, TRANSPORTATION, MEDICATIONS
Need transportation, help with shopping? Need someone to pick up mail or medications? Call us now!

SAFE DISPOSAL OF MEDICATIONS
Community Connections participates in a system called "TAKE IT TO THE BOX!" We carry boxes, pouches, and disposal systems for prescription medicines. These items are available for FREE and can be picked up at our office.

VITA TAX SERVICES
Call for assistance with tax preparation and services.
